    目的 观察12周普拉提及躯干力量训练对慢性非特异性腰痛(CNSLBP)女大学生疼痛视觉模拟评分(visual analog scale,VAS)、腰痛功能障碍评分(Roland-Morris Disability Questionnaire,RMDQ)及躯干屈伸肌力和动态平衡能力的影响,为CNSLBP女大学生制定康复干预提供实践依据。方法 将45名CNSLBP女大学生随机分为普拉提组22名(进行12周普拉提运动干预,3次/周)和力量训练组23名(进行12周躯干力量训炼)。运动结束后,测试所有受试者运动前、后VAS及RMDQ评分、躯干肌力(屈肌峰力矩,FPT;伸肌峰力矩,EPT;屈/伸肌峰值力矩比,F/E)及动态平衡参数(总体稳定指数,SI;前后方向稳定指数,APSI;左右方向稳定指数,MLSI)。结果 1)与运动前比较,经过12周不同运动干预,力量训练组及普拉提组CNSLBP女大学生VAS及RMDQ分值均显著下降(t值分别为5.527,8.775,10.577,11.198,P均<0.01);30o/s及90o/s 角速度躯干FPT及EPT均显著增加(FPT:t值分别为-2.744,-3.309,-4.074,-2.980;EPT:t=-6.876,-11.707,-6.961,-6.400,P<0.01,P<0.05),F/E 比值(30o/s及90o/s)均显著下降(t值分别为5.258,6.810,4.283,2.654,P<0.01,P<0.05);睁眼状态下仅普拉提组NSLBP女大学生APSI显著下降(t=2.808,P<0.05);闭眼状态下普拉提组NSLBP女大学生SI、APSI及MLSI均显著下降(t值分别为7.718,7.351,7.804,P均<0.01),力量训练组仅APSI显著下降(t=3.395,P<0.01)。2)12周运动干预后,与力量训练组比较,普拉提组VAS及RMDQ分值、F/E 比值(30o/s及90o/s)均显著下降(t值分别为5.571,2.812,2.576,2.339,P<0.01,P<0.05)、30o/s及90o/s 躯干伸肌肌力显著增加(t值分别为-3.974、-3.927,P均<0.01);闭眼状态下SI、APSI及MLSI均显著低于力量训练组(t值分别为6.309,3.019,2.719,P均<0.01),其他指标两组之间比较均无统计学意义(P均>0.05)。结论 与躯干力量训练比较,普拉提运动可显著增强及平衡躯干屈伸肌力,增强动态平衡能力,改善CNSLBP女大学生下腰背痛。

    Abstract:

    objective To observe the effects of pilates exercise and trunk strength training on the scores of VAS (visual analogue scale) and RMDQ (Roland-Morris Disability Questionnaire), trunk extension and ?exion strength and dynamic balance, which offers a practical-based evidence for the rehabilitation treatment for female college students with CNSLBP. Methods 45 female college students with CNSLBP were assigned to pilates group (n=22) and strength training group (n=23); the subjects in pilates group underwent pilates exercise for 12 weeks (3 times per week) and the strength training group underwent trunk strength training 12 weeks (3 times per week). Then, the scores of VAS and RMDQ, trunk strength (FPT, EPT and F/E) and dynamic balance parameter (SI, APSI and MLSI) were measured before and after exercise. Results 1) After 12 weeks exercise, compared with the pre exercise, the scores of VAS and RMDQ decreased (t=5.527, 8.775, 1.577,11.198, P<0.01 respectively); the FPT and EPT at the 30o/s and 90o/s angular velocities significantly increased (FPT: t=-2.744, -3.309, -4.074, -2.980, t=-6.876, -11.707, -6.961, -6.400, P<0.01, P<0.05) and the ratio of F/E (30o/s and 90o/s) decreased (t=5.258, 6.810, 4.283, 2.654, P<0.01,P<0.05) in the two groups; in the eye opening conditions, only APSI significantly decreased (t=2.808, P<0.05) in pilates group. in the eye closed conditions, SI, APSI and MLSI significantly decreased (t=7.718, 7.351, 7.804, P<0.01 respectively) in pilates group and only APSI decreased (t=3.395, P<0.01) in the strength training. 2) After 12 weeks exercise, compared with the strength training group, the scores of VAS and RMDQ decreased (t=5.571, 2.812, P<0.01); the 30o/s and 90o/s EPT significantly increased (t=-3.974, -3.927, P<0.01 respectively), the ratio of F/E(30o/s and 90o/s ) and SI, APSI and MLSI significantly decreased (t=2.576, 2.339, 6.309, 3.019, 2.719, P<0.05, P<0.01), there was no statistical difference in other indexes between the two groups (P>0.05 respectively). Conclusions An 12-week pilates program enhanced the trunk strength and dynamic balance ability, improved the imbalance of trunk extension and ?exion strength and ameliorated the low back pain more than a trunk strengthening exercise program among female college students with CNSLBP.
