泰森多边形多孔结构髋臼杯初始稳定性研究

    目的 分析泰森多边形多孔结构髋臼杯与实体髋臼杯的初始稳定性差异,探究泰森多边形结构多孔层对髋臼杯初始稳定性的影响以及其在预防松动脱位方面的作用。方法 通过Grasshopper设计孔隙率分别为60%、70%的泰森多边形多孔结构支架,利用选区激光熔化技术(selective laser melting,SLM)制备实体髋臼杯与孔隙率为60%、70%的多孔髋臼杯样件各3个,在相同条件的聚氨酯块模型中进行杠杆试验,对得出的最大撬出力矩、偏转角度、界面刚度进行分析对比。结果 在压入力大小无明显差异的情况下孔隙率为60%、70%的多孔髋臼杯的最大撬出力矩分别比实体髋臼杯高278.82%、320.56%,偏转角度比实体髋臼杯高194.04%、269.23%,界面刚度比实体髋臼杯高18.58%、7.88%。在杠杆试验完成后,多孔髋臼杯所用的聚氨酯块半球腔内出现明显磨损。结论 泰森多边形多孔结构髋臼杯的初始稳定性指标均高于实体髋臼杯,说明泰森多边形结构多孔层能提高髋臼杯的初始稳定性。研究结果能够对髋臼假体的设计与选型提供一定的参考。

    Abstract:

    Objective To analyze the differences in initial stability between the Voronoi polygon porous structure acetabular cup and the solid acetabular cup, and explore the impact of the Voronoi polygonal structure porous layer on the initial stability of the acetabular cup, as well as its role in preventing loosening and dislocation. Methods Voronoi polyhedron porous scaffold structures with porosity of 60% and 70% were designed using Grasshopper. Utilize selective laser melting (SLM) technology to manufacture three samples each of solid acetabular cups and porous acetabular cups with porosity of 60% and 70%. Conduct lever-out tests on the acetabular cups using polyurethane block models under identical conditions, analyze and compare the resulting maximum lever-out torque, angular displacement, and interface stiffness. Results For porous acetabular cups with porosity of 60% and 70%, and with no significant difference in applied insertion force, the maximum lever-out torque is 278.82% and 320.56% higher, respectively, compared to solid acetabular cups. The angular displacement is also higher, with increases of 194.04% and 269.23%, while the interface stiffness is elevated by 18.58% and 7.88% compared to solid acetabular cups. Significant wear was observed within the polyurethane block hemisphere cavity after completing the lever-out tests using the porous acetabular cups. Conclusion The initial stability indicators of the Voronoi polyhedron porous structure acetabular cup are higher than those of the solid acetabular cup, indicating that the Voronoi polygonal structure porous layer can enhance the initial stability of the acetabular cup. The research results can provide some reference for the design and selection of acetabular components.
