    目的 本研究探索病变后主动脉夹层血流动力学性能,为患者下一步治疗提供更加科学的依据。方法 基于一例复杂Stanford B型主动脉夹层患者的CTA影像数据建立个性化主动脉夹层近心端不同破口形态(H型、O型、V型)的夹层模型,结合计算流体动力学(computational fluid dynamics,CFD)与形态学分析方法,分析破口截面速度分布、血流状态分布、壁面压力分布以及壁面剪应力分布。结果 H型破口类型在破裂入口处的流速、最高压强差、壁面剪应力占比都表现出较其他两种类型较大的血流动力学参数,获得了主动脉夹层血流动力学参数与破口类型的相关性。结论 H型破口类型夹层破裂风险最大,V型次之,O型最小,为病例进一步的数值分析和治疗方案的制定提供有效的参考依据。

    Abstract:

    Objective This study explores the hemodynamic performance of aortic dissection after lesions and provides a more scientific basis for the next treatment of patients. Methods Based on the CTA image data of a patient with complex Stanford B-type aortic dissection, a personalized aortic dissection model with different fracture shapes (H-type, O-type, and V-type) at the proximal end of the aortic dissection was established, combined with computational fluid dynamics (CFD) and morphological analysis methods to analyze the velocity distribution of the fracture section, the velocity distribution of the flow path, the wall pressure distribution and the wall shear stress distribution. Results The flow velocity, the highest pressure difference, and the proportion of wall shear stress at the entrance of the H-shaped rupture showed larger hemodynamic parameters than the other two types. The hemodynamic parameters and rupture parameters of the aortic dissection are related to the type of mouth. Conclusion The risk of dissection rupture of type H fracture is greater, type V is the second, type O is the smallest, which provides an effective evaluation basis for further numerical analysis of cases and formulation of treatment plans.
